Under the Early Years Services Regulations (2016), early years services are required to record any accident, injury or incident involving a child attending the service (Regulation 16 Record in relation to a preschool service).

Any care given in a first aid situation must be documented in line with the service's policy on accidents and incidents (Regulation 23 Safeguarding Health, Safety and Welfare of Child).

An accident is an unpredictable and therefore, unavoidable event. Also known as an unintentional injury.

An incident is an unusual, unplanned event that may result in injury, illness or damage or disruption of normal service operations.

Following each accident or incident, a risk assessment should be carried out. The actions must document changes made to prevent the accident or incident re-occurring (Regulation 23 Safeguarding Health, Safety and Welfare of Child, Regulation 31 Notification of Incidents). This risk assessment must be documented.

Under Regulation 31 you must give feedback to parents and guardians after your risk assessment has been carried out.

The forms in the Accident and Incident book are in triplicate; one form should stay in the book, one copy should be given to the parent/guardian and one copy should be stored on the child's file in the service. There are 30 (triplicate) forms in total.

Accident/Incident Book

I my opinion this book is a must have. In the past I used to record incidents in a notebook but it is difficult to remember to record everything. With this book, each section requires the recorder to note valuable pieces of information about the incident. Parents, witness and person-in-charge sign the incident and the original record can be torn out along perforated lines and given to the parents whilst the preschool retains a carbon copy in case it's needed in the future.

My only criticism is that we seem to record the name of the service twice on the form which is unneccessary.

This book will help you to log your accidents and incidents, it has a copy for you and a copy for the parent.
